Investors, who have changed their name due to marriage or otherwise, would have to record the same in their folios. Similarly, if the name in the application form is inadvertently filled incorrectly, the investor would need to get the same rectified.

The requirements in this regard are given below. These may marginal variance in the procedure from fund to fund. Investors who have changed their name and recorded the same in the Gazette should submit the below documents to the mutual fund:

A request letter duly signed is required. The signature of the investor in the letter should be attested by the Bank Manager. A copy of the Gazette notification attested by a Notary Public shall be submitted.

A copy of the PAN Card reflecting the new name should also be attached. This would mean that the investor has got the name change effected in the records of Income Tax before submitting the request to the mutual fund.

Proof of change in name in the bank account must also be produced. Investors should submit an original cancelled cheque leaf in which the unit-holder's name is printed or a copy of the bank statement, passbook duly attested by the bank manager. The name should match with the name in the request

Change through CVL

Some mutual funds ask for an indemnity bond on a stamp-paper of Rs 300. The format of the Bond would be available with the mutual fund

Investors should get the name change effected with CVL. Change request forms are available at the CVL website and investors should fill in the form, specifying the change in name and submit the required documents to CVL. A change in name after marriage would require the following records:

- A written request duly signed (with both old and new signature or new signature attested by the existing banker on record)

- Notarised copy of marriage certificate

In the absence of a marriage Certificate, an affidavit on stamp paper for Rs 100 may be submitted. The format of this affidavit can be obtained from the mutual fund. A copy of the PAN with the PAN number being the same as before and the name change alone reflected in the PAN.

KYC acknowledgement in new name (the investor should have got the name change effected with CVL first) should also be accompanied.

Errors in filling forms

If the correction is due to application form being filled incorrectly, investors may send a request letter duly signed by the investor along with a copy of the PAN. The PAN number should be the same as the one registered in the folio. The KYC should be in the correct name as requested by the investor with proof of KYC acknowledgement form / verified status in the CVL website. Some mutual funds ask for an Indemnity Bond on plain paper, the format of which is available with the fund.

Other changes

Investors who desire to include middle name / expansion of initial may do so by sending a request letter duly signed and submit a certificate from the Banker, being the Banker on record in the folio, confirming the account holder's full name.

A reference will be made to the copy of the PAN to verify the full name. Some funds may ask for an affidavit notarised in original on Rs 100 non-judicial stamp paper stating the old and new names of the person and also clearly specifying that both the names refer to one and the same person.
